Case Manager - Developmental Disabilities Services

Overview

Our social services organization is looking for a dedicated and empathetic Case Manager to coordinate care and support adults with developmental disabilities. This professional position involves developing individualized plans, overseeing client services, and collaborating within a supportive team to promote greater independence for clients.

Primary Duties

• Review and analyze assessment data to create comprehensive Individual Service Plans (ISPs).
• Coordinate and attend meetings with interdisciplinary teams.
• Prepare Semi-Annual Progress Reports and maintain accurate client case files.
• Train and support direct care staff in implementing goals and maintaining records.
• Provide job coaching assistance as needed.
• Ensure all documentation meets confidentiality standards and regulatory requirements.
• Monitor client progress and adjust services to meet evolving needs.
• Serve as a liaison between clients, their families, and service providers.
• Conduct necessary field visits with clients.
• Perform additional tasks as assigned.

Required Qualifications

• A Bachelor's degree is required, with a preference for Human Services or related disciplines.
• Two years of prior experience in developing and implementing vocational plans within a program setting is preferred.
• Background in creating and applying behavior modification intervention services is a plus.
• Direct experience working with individuals with developmental disabilities.
• CPR and First Aid certification (must be obtained within 30 days of hire).
• A clean criminal background check, meeting DDS licensing requirements (LIC 508).
• Access to reliable transportation and a valid California driver's license.
• Current auto insurance meeting state minimums.
• Solid understanding of behavior modification principles and human development.
• Strong organizational skills and the ability to work independently.
• Exceptional written and verbal English communication skills (Spanish language proficiency is a bonus).
• Proven ability to collaborate effectively within an interdisciplinary team.
