Case Manager - Create Meaningful Change for Adults with Disabilities

Position Summary

Our social services organization is looking for a compassionate and committed Case Manager to assist adults with developmental disabilities. This professional, full-time position involves coordinating services, designing personalized support strategies, and serving as the main advocate for clients striving toward increased self-sufficiency. The role provides an attractive salary and a comprehensive benefits package following a 60-day probation period.

Key Responsibilities

• Review and analyze assessment data to develop comprehensive Individual Service Plans (ISPs).
• Arrange and attend meetings with multidisciplinary teams.
• Prepare Semi-Annual Progress Reports and ensure case files are maintained accurately and up to date.
• Train and support direct care staff in executing objectives and preserving proper documentation.
• Provide job coaching support when required.
• Ensure all records comply with confidentiality policies and regulatory standards.
• Monitor client progress and adjust services to meet evolving needs.
• Serve as a liaison between clients, their families, and various service providers.
• Complete required field visits with clients.
• Perform additional tasks as assigned.

Qualifications

• A Bachelor's degree is required, with a preference for Human Services or a related discipline.
• At least two years of experience writing and implementing vocational plans in a program setting is preferred.
• Experience developing and applying behavior modification intervention services is advantageous.
• Direct experience supporting people with developmental disabilities.
• CPR and First Aid certification (must be obtained within the first 30 days of employment).
• A clean criminal background check meeting DDS licensing requirements (LIC 508).
• Access to a reliable vehicle and a valid California driver's license.
• Current auto insurance meeting state minimum coverage levels.
• Solid understanding of behavior modification principles and human development.
• Excellent organizational abilities and the capacity to work independently.
• Outstanding written and verbal communication skills in English (Spanish proficiency is a plus).
• Demonstrated ability to collaborate effectively as part of a multidisciplinary team.
