Position Summary
APLA Health is dedicated to delivering high-quality, compassionate medical care and supportive services. We are currently looking for an Intensive Case Manager (ICM) to support clients who are experiencing homelessness and living with a chronic illness or physical disability, guiding them through all phases of housing stability. This hybrid/remote role requires a valid California driver’s license and access to a personal vehicle.
Qualifications
- A Bachelor’s degree is mandatory.
- At least one year of experience working with homeless individuals.
- Strong command of Microsoft Office Suite, along with excellent written and verbal communication abilities.
- Understanding of HIV/AIDS-related matters, chronic homelessness, co-occurring conditions (such as mental illness and substance abuse), and challenges faced by low-income, multi-ethnic populations.
- Capability to work both independently and collaboratively, with strong organizational skills, attention to detail, and the ability to manage multiple priorities and deadlines.
- Must hold a valid California driver’s license, provide proof of auto liability insurance, and have a personal vehicle available for work-related travel.
- COVID-19 vaccination or an approved medical or religious exemption is necessary.
Core Duties and Responsibilities
- Verify client eligibility and complete intake procedures.
- Carry out comprehensive psychosocial evaluations and create individualized service plans.
- Help clients access temporary and permanent housing, including submitting applications for Section-8 and other housing programs.
- Offer housing location support, assist with lease agreements, and coordinate move-in logistics.
- Maintain consistent in-person contact with clients, including home visits and accompanying them to appointments.
- Ensure clients are connected to medical, mental health, substance use treatment, and other necessary support services.
- Assist clients with budget management, securing benefits (such as SSI and SSDI), and finding employment or educational opportunities.
- Provide transportation aid and help clients meet immediate needs like food and clothing.
- Educate clients on tenant rights and obligations, and implement eviction prevention strategies.
- Keep detailed and organized client files, case notes, and database entries.
- Manage a caseload of 20 to 40 clients, depending on their level of need.
