Clinic Embedded HIV Engagement Specialist (CEHES)
Status: Full-time, Non-Exempt
Schedule: Monday – Friday, 8 AM – 5 PM
Location: On-site
Bilingual (English/Spanish): Yes: A Spanish assessment is required before an offer can be made.
This position offers a rate of $26.37 - $29.68 per hour. Compensation will be determined based on experience.
Overview
Reporting to the Data to Care (D2C) Program Manager, the Clinic Embedded HIV Engagement Specialist (CEHES) will focus on patient outreach, engagement, and re-engagement efforts to connect individuals living with HIV (PLWH) to medical services. This role is a field-based hybrid position and is part of a three-year pilot program in Los Angeles County.
Requirements
- Training and Experience (Option A): A Bachelor’s degree in Social Work, Psychology, or a related discipline, combined with at least one year of experience serving LGBTQIA+ PLWH.
- Training and Experience (Option B): A high school diploma (or equivalent) and at least three years of experience serving LGBTQIA+ PLWH.
- Background in a nonprofit setting and specific training or experience working with people living with HIV/AIDS.
- Experience engaging with culturally diverse populations.
- Capability to provide crisis intervention support.
- Bilingual required – English/Spanish.
- Must have a valid California driver’s license, proof of auto liability insurance, and access to a personal vehicle for work-related tasks.
- COVID-19 vaccination and booster, or an approved medical/religious exemption, is required.
Essential Duties and Responsibilities
- Develop, coordinate, implement, and assess patient retention strategies for individuals with HIV/AIDS who are out of care.
- Perform outreach activities, including in-person field visits and home visits.
- Facilitate referrals and connections to medical care and ancillary support services.
- Build partnerships with community organizations to create referral pathways.
- Record and update program data and maintain clinical documentation.
- Deliver HIV education, including information on treatment options.
- Communicate effectively with clients and patients in both English and Spanish.
