Social Worker
Status: Full-Time, Non-Exempt
Schedule: Monday to Friday, 8:00 AM – 5:00 PM
Location: On-Site
Compensation: This role offers a rate of $27.60 – $32.80 per hour. Salary is based on relevant experience.
Overview
APLA Health delivers high-quality, compassionate medical care as a patient-centered health home, combining services in medicine, dentistry, behavioral health, HIV treatment, and support programs. We are currently looking for a Social Worker to deliver coordinated care to individuals living with HIV and other chronic illnesses. Requirements
- A Bachelor’s degree is required in Social Work or an equivalent discipline.
- At least one year of experience working as a Social Worker; experience in Internal Medicine or Family Medicine is preferred.
- Familiarity and experience with target populations, such as intravenous drug users and sexual minorities, are advantageous.
- HIPAA certification and understanding of informed consent procedures and medical terminology.
- Excellent assessment, evaluation, and problem-solving capabilities.
- Capacity to work effectively with diverse teams, uphold confidentiality, and manage multiple tasks in a fast-evolving environment.
- Proficiency in using computerized patient care systems.
Special Requirements
- Proof of COVID-19 vaccination and booster, or an approved Medical/Religious Exemption is mandatory.
- Occasional weekend or evening shifts may be necessary.
- Local travel within Los Angeles County may be required.
